Retinol is generally safe for topical use but can cause irritation, peeling, redness, and photosensitivity especially in the initial phase or at higher concentrations. Gradual introduction, proper formulation, and nighttime use with sunscreen protection during the day mitigate side effects. Its instability in light and air necessitates stabilized formulations.

Retinol is a fat-soluble vitamin A derivative that penetrates the stratum corneum and reaches the basal layer of the epidermis and superficial dermis. Inside keratinocytes, it is converted to retinoic acid, which binds to nuclear retinoic acid receptors (RARs) and retinoid X receptors (RXRs), regulating gene expression related to cell proliferation, differentiation, and collagen synthesis. Retinol stimulates keratinocyte proliferation, promoting epidermal thickening and enhancing barrier function. It reduces transepidermal water loss (TEWL), protects collagen from degradation by inhibiting matrix metalloproteinases (MMPs), and stimulates new collagen production to improve skin firmness and elasticity. Retinol also accelerates exfoliation, promoting smoother skin texture and hormone-like regulation of skin renewal. Compared to prescription retinoids, retinol is less potent but still effective with less irritation.
